You will need to have the test results available, in order to follow the set-up steps in the article below.

You will first need a test result showing "mild to moderate" hearing loss.

(ie: For "serious" hearing loss, seems best to consult with a hearing-professional.)

Although the video below is for setting up hearing health features on iPhone,

it may help to familiarize you with the similar "set-up" on Mac, now that the features can also be used on Mac.

Sorry I don't have any answer to that particular situation ... you could try chatting with Apple Support, or asking at your nearest Apple Store ... maybe they have a way to help with the set-up ?
